Medline bull case meets real operating tests

Bulls argue Medline is building a higher quality, more efficient growth engine through automation, Prime Vendor wins and supply chain expansion. Q2 backs some of that up. Net sales rose about 12% to US$7.7b and management lifted full year organic growth guidance to a range of 9% to 10%, helped by Supply Chain Solutions at US$4.1b with 16% growth and Medline Brand at US$3.5b with 7% growth despite retail pressure. Prime Vendor traction looks real, with more than US$650m of new customer signings in the first half, already around 65% of the US$1b full year goal. Automation and capacity projects are progressing, including more AutoStore deployments and a larger California footprint after the Tracy fire. However, the need to cut adjusted EBITDA guidance to US$3.3–3.4b shows efficiency gains are not yet offsetting higher input costs and quality and investment spend.

Margin reset and regulatory overhang fuel bear worries

Bears focus on thin profitability, execution risk on big projects and regulatory and legal exposure. Q2 offers some clear support for those concerns. Net income excluding extra items fell sharply to US$60m against US$333m a year ago, despite the IEEPA tariff refund that lifted reported adjusted EBITDA to US$1.06b. Underlying adjusted EBITDA of about US$817m plus the cut to full year EBITDA guidance signal a more expensive cost base from fuel, inputs, quality remediation and California distribution rebuilds. Management expects roughly half of the incremental cost impact to be ongoing, which points to a structurally lower margin starting point. The FDA warning letter and related securities investigation remain unresolved and management flagged higher Medical Device Reports and remediation expenses in the second half of 2026. The stock’s near 13% one day drop and weaker 30 day and 90 day returns show these risks are front and center for investors.
